In D, although it seems more parallel due to :

The "and" before "spending" makes the construction awkward

Is that correct? It is not the increase that amounts, but the efficiency...

d). that impose stricter limits on medical services and require doctors to see more patients, and spending
if there is no typo, it is clearly not correct. What is the first part for // of ..... AND spending?
without the AND, wd be correct.
