I would say the structure is: has been blamed for a) causing hundreds of deaths, injury to thousands, and b) seriously damaging the nation's recreational areas. Better would be "causing hundreds of deaths and injury to thousands", but since this option is not given I stick with A.

Even though I agree w/ the parallelism in D, but doesn't "as well as" change the meaning of the context?

Yes, i do know "as well as" similar to "and". However, as I recalled one of the few past examples from Verita (maybe I cant remember exactly), there was a slightly a change in meaning when you switch from "and" to "as well as". That was why I hesitated.
