E it is..
(A) it was only the second published by an American woman
- no need of unnecessay indepnedent clause.
(B) it was only the second that an American woman published
- no need of unnecessay indepnedent clause.
(C) the second one only published by an American woman
- misplaced only..
(D) the second one only that an American woman published
- again same poroblem and weird structure
(E) only the second published by an American woman
- keep this one.

E for me. compared with C, E sounds less wordy while still holding the sentences true meaning.

OA is E. C is plain awkward when it places 'only published' after 'the second'
