A and B don't fit because of the it. Only E supports parallelism.

I found D as awkward. It has a misplaced only and an unnecessary one.

first and the second is parallel.

only is the adverb refering to the adjective second.

in D one is unneccessary. and that is awkward.
if it were the second only by an American woman published , it would be correct.
So E is the only right choice.
