A and B-- use of it seems ambiguous..
C-and D- use of only does not seem right..

E seems right

A) it was only the second published by an American woman
"it"..Pronoun error
(B) it was only the second that an American woman published
Not Parallel
(C) the second one only published by an American woman
Changes the meaning of sentence..Seems like only american women can publich it
(D) the second one only that an American woman published
No parallel
(E) only the second published by an American woman
