The second paragraph starts off seemingly to be "A"'s answer but if read further it is supporting first paragraph by giving more info/examples.

The second paragraph introduces "However" clause that might seem to contrast with the explanation of the second paragraph.
But if we read further into the details, the second para also provides an example to clarify TQM model.
Thus, if we need to provide an option that includes above two cases that is best supported by option D.

Please correct me if I am wrong.
