D is best here with the most correct parallelism:

1. be blamed for causing A and B as well as doing C
2. A and B are parallel, too.

D, parallelism:

they have also been blamed for causing hundreds of deaths and thousands of injuries, as well as doing serious damage to the nation’s recreational areas.

(A) deaths, injury to thousands, and seriously damaging
(D) deaths and thousands of injuries, as well as doing serious damage to

"injury" as a singular noun in A is incorrect ...it should be " injuries to thousands ( thousands of what?!!)"
D has the best parallelism.
