It was between C and D for me. I picked D for the following reasons
Though I liked C for parallelism I found it awkward because it says "an investigation........has found that all twelve consistently break the laws, that many residents work longer than 24 hours straight, and that more than half the surgical residents work"
"investgation....has found" is awkward compared to "state medical officials who found" (D)
whereas D says.....
twelve hospitals were investigated by state medical officials who found all twelve breaking the laws, with many residents working longer than 24 hours straight, and more than half the surgical residents work
I would like expert comments on this.

IMO C for the reason stated by nusmavrik.
D is not parallel in construction - twelve hospitals were investigated by state medical officials who found all twelve breaking the laws, with many residents working longer than 24 hours straight, and more than half the surgical residents work

In B, the subject is investigation; "by state medical officials" and "of twelve hospitals" immediately following the subject (investigation) are preopositional phrases and hence the verb (have found) becomes incorrect because being plural it can't go with the singular subject "investigation". I immediately ruled out B because of this even though I understood that it was the officials who were doing the finding and not the investigation. Now while whether INVESTIGATION can actually FIND something might be debatable - all other choices apart from D break parallelism and have some grammatical error or the other. The only issue with D was whether Investigation can actually DO the finding. Does B contain also an error of parallelism?, or just S-V agreement and change of the meaning? In C, when you have to make a list of clauses parallel, can you omit the first word (that) in the second and the rest of clauses? Why does ykaiim mention that "a sub-clause starting with with+.....verb+ing is always wrong"? I think that there is not a clause; there is a phrase.

C it is... A and D are out because the initial sentence should modify "an investigation" and not the hospitals... B- found requires "that".. Also "has" is required and not "have" between C and E.. sentence structure in C is more parallel than E.. Moreover E has modifier issues.. When "of twelve hospitals" is removed, the sentence does not make sense...

I thought of D. 'Investigation...has' investigation is already finished so isn't it should be 'investigation...had'?

C is good in parallelism "found that..., that..., and that..."
active voice is preferred in GMAT to passive voice
and "an investigation...has found..."
