The logic of the sentence requires contrast.
Although X, Y.
This is best represented by answer choice B IMO. The logic and intent is very clear.

Correct Choice is B. Here is my explanation.

This Sentence has following elements.
Although………cousins(subordinate clause states the fact about subject)
heirloom tomatoes (subject)
grown from….year (modifier)
heirlooms are……demand (main clause states some another fact about subject)

Main subject has came twice in this sentence. so we would need a clause between them. the ununderlined portion is (Subject)(Clause) so the accepted word order for the underlined portion will be (Contrast word)(subject)(Modifier)(Clause). Also the verb appearing should be in simple present appear because here we are discussing general state about heirlooms. i.e. they regularly appears less appetizing

A) Wrong structure (Clause)(Subject)(Modifier). Also appearing is wrong in the context of sentence.

B) (Contrast Word)(Subject)(modifier)(subordinate clause) correct order. Appear is correct

C) (Clause)(Subject)(modifier). Incorrect

D) (Modifier)(Subject)(Clause) here the contrast word has been ommited. So the sentence does not convey intended meaning.

E) (Subject)(Modifier)(clause). Here by bringing the main subject at front position they have made its use in second clause unnecessary ; their you would need its pronoun. Also they in although……cousinsis redundant.
