(A) while being an adolescent - "being" wrong
(B) while in adolescence - it is better to avoid "While". Further, She signed as an adolescent has clairty
(C) at the time of her being adolescent - "being" wrong
(D) as being in adolescence - "being" wrong

A is wrong because " while" and "being" make the sentence wordy. You don't need both to convey the meaning
C and D are wrong again because they both are wordy
E changes the meaning of the sentence. It is 'in' adolescence that she signed but not 'as' an adolescent.

B conveys the meaning in the most concise form

I chose B too..
But OA is E.

I got it now..

she had signed .. (pastense)... while she was in adolescence.. (second part must be in past tesne right?)

Then only E remains.

I understand what you meant when you said that in E there is a change in meaning. When you sign something as an adolescent, it can be understood that you signed something as an adolescent would sign (carelessly, for example).

I chose E, but I hadn't seen it that way...

as an adolescent, I did xyz

as an adult, I started drinking alcohol

I just dont see how while in adolescence even comes close.
