All options look wrong..

D would have been correct.. if "and spending" is "spending"

that impose stricter limits on medical services and require doctors to see more patients, and spending

that impose stricter limits on medical services and require doctors to see more patients, spending
in this case spending modifer to "see more patients"

sub - Insurance plans..

Plans .. impose.. require.. (correct) but plans can't spend.. so "AND" is wrong in all choices.

In for A

end insurance plans imposing stricter limits on medical services and requiring doctors to see more patients, and [to - understood ] spend less time with each.

The only thing which worries me is the last comma ( in red ).
