'E' it should be.
It talks about 'the plan' that imposes stricter limit on services by requiring doctors see more patients & spending less time/patient.

E definetly is better than the rest.
C is a trap which seems to indicate thet Insurance plan spends less time with each.
E correctly says that Plans are requiring doctors to spend less time..

The OA is C. If there is a comma before and spend, this sentence is perfect.

You are kidding us. C can not be the correct choice.
Try breaking the sentence.
Among lower-paid workers, union members are less likely than nonunion members to be enrolled in lower-end insurance plans
it does 3 things.
1. plans THAT impose stricter limits on medical services,
2. plans THAT require doctors to see more patients,
3. and plans THAT spend less time with each
in 3rd, what is EACH referring to. I think it should refer to patients. Right.
Now ATLEAST 2 problems with this question.
1. The plan can not spend time with each.
2. To be parallel, we need repetition of word THAT after each predicate. Check OG question (many of the dioxins that are uncontrolled and that north americans are exposted to. Choice C, which is wrong in that sentence too because of missing THAT)
Anyway... I hope I dont see these type of questions in real GMAT.

I choose C, then i thought E after reading ritesh and others but after reading the OA, now i am confused..and a little happy...

i am not agreeing with C either. I think its not parallel , what does the OE say and where is this quesiton from?
