"A"....use of non-restrictive clause is good here, begun 2 decades ago is additional info without which the sentence still stands.

i think it should be E for correct idiom usage Attempt to

we need 'ratify equal rights amendment' to come after attempt, so that is properly modified by the last phrase 'has been unsuccessful despite efforts by many important groups, including the National Organization for Women'. 'begun almost two decades ago' only serve to provide more information regarding the attempt - a non-essential phrase

attemp to is idiomatic, so A it is (over D)
