C for me
Any medical test will sometimes fail to detect a condition when it is present and indicate that there is one when it is not.

(A) a condition when it is present and indicate that there is one
(B) when a condition is present and indicate that there is one
(C) a condition when it is present and indicate that it[refers to condition] is present..
(D) when a condition is present and indicate its presence
(E) the presence of a condition when it is there and indicate its presence->awkward construction

Take a look at the last part of the sentence..."when it is not"
Only C is parallel to this.
The last part of A & B should be: "when there is not"
D & E use "presence" - not parallel.

I choose C too. C is only choice makes sentence parallel.

I would say E.
It's not C because the "it" is ambiguous; we cannot tell if it refers to the medical test or the condition.
Any medical test will sometimes fail to detect a condition when it is present => what is present? the medical test or the condition?

I think E, although wordy, conserves the parallelism:
"will fail to detect the presence of a condition when it is there and indicate its presence when it is not."
