Go for D.

The reason is that you are comparing the way children form their perception to the way that adults do.

The original sentence is comparing the way children form their perspections to adults, which does not make sense.

Another vote for D.

When comparing Nouns use "Like" and when comaring action use "As"
But there is a very thin line between the two. One has to read the sentence and understand what is being conveyed. Here clearly actions are compared so it is advisable to use "as"
The sentence begins with a prepositional phrase that conveys action.

For example -
Like adults, children cannot differentiate between reality and fantasy.
Here the attributes of the children and adults are being compared.
