The trick is that only A correctly uses an idiom "both-and". Even so, your answer is OK.

questionable.... it is still questionable...
one of the substances that tell/tells --- difficult to distinguish one from the other.

I disagree.
I stood under one of the many clocks that adorn the city of London.
I shot one of the pigeons that infest the corners of my barn.
I am one of the instructors who love to give a lot of homework.
Aids is just one of the many diseases that cause premature death.

I have to agree:
a pronoun must modify the CLOSEST word

Will One of the Substances -- needs singular verb or plural verb ??

would go with D. because "One of" the substances require singular verb "tells" and both x and Y - x and y should be parallel.
Its simple - did scientist find more than one substance that does the action or did they find "One Substance"
If they found more than one then plural verb which is not the case.
"One of" will usually require a singular verb after. Unless there is a intruder, splitting the independent clause such as "that" then the verb can be either singular/plural.
From notes collected from Gmat club:
One of the: The pattern to remember is ‘one of the NOUN (this noun will always be plural) + that/who + PLURAL VERB
Over looked the word "That" - answer is A

This is not necessarily true.
One of the boxes is open.
It depends on the sentence. For the current discussion the relative pronoun that modifies substances, hence the verb is plural.

Not always the case......
One of the students believe/believes that math is boring..
Here only One believes that math is boring.
its "believes" - singular...
Another example:
He is one of the students who believe that math is boring...
In this case there are many students who believe math is boring and

two things are required here:
1) one of the + nouns + that + plural verb
2) both...and....
