I have Original copies of Provisional Degree Certificate, Each semester's mark sheets and transcript. I have a scanned copy of my degree certificate too. I need a detailed report for BYU.

3. My transcript does not have the number of hours mentioned in its report. How do we get them the syllabus from university ?
4. Do we ask for the report to be sent to our address or to be reported directly to BYU ?

I can understand your situation. I was also going through the same in September. Before going to your point wise questions, I would say that you need to send Original Degree certificate as they do not consider either Original Provisional Degree Certificate or Scan copies of Degree. I had to send then separately my degree certificate in the second time as I was too scared to send it in the first go. If you do not send the Original they will ask you if you want them to go ahead without it but they will not mention that your degree is evaluated. Said that I will advise to send original degree as well.
Now your questions:
1. Once you have paid the fee, IERF will send you a mail with two attachment. One of the attachment will have the address on it that you need to paste it on your Envelop.
NOTE: Don't forget to ask your courier to take a scan of that page and place it on the top of the packaging they do over the Envelop.
2. They have clearly mentioned in their site which all documents you need to send and/or additional documents depending upon your nationality.
I am an Indian candidate and have sent the below documents in Original
a) All Postgraduate/Undergraduate degree certificate
b) Transcript + each semester mark sheet (although anyone will work)
c) In case, any of your above degrees is not in English language, you need to send them a authenticated English translation.
d) Higher Secondary degrees (Although not required but I didn't want to take risk so send them all)
e) One xerox copies of each Original document you are sending.
3. No# of hour will be calculated by IERF in their evaluation report ( in #NOTE1 section). I believe that will be automatically taken ( along with GPA value) by BYU guys when they will receive their copy of your result.
4. IERF will automatically know once you have mentioned the BYU as the university in IERF application form. They will also send you an Applicant copy of evaluation result along with your original certificates.
