It seems that most scholarship applications ask for one of these.

Can anyone who has done one before post an example?

Is a table enough, or does it require an explanation as well??
How detailed do these things need to be? Some ask for details on income from investments. How can this be taken into account unless those investments are sold?

Income from investments can be many things. Among of the most common are Dividends, Rental Income and Interest Payments.

I haven't filled one out, so I don't know how in depth you need to go.
