I believe E is better. In the first case, the pronoun them could refer to either foods or patients.

I dont think that " them" can refer to patients in A. It will be going too far. Iam sure their is some other issue here.. may be of brevity and alternate use of word "it"

Yes E is correct..but can anybody explain the use of "it" here.

I believe (E) is better for two reasons:

First and foremost is the parrallelism: not only by what patients eat but also by when they eat it.

Here two bold parts they make the sentence parallel.

Second is the clear reference of "it", correctly refers to the food.
While "foods.......them" looks bit awkward.

'it' here refers to 'whatever' the patients eat. 'it' here might be labelled as redundent, but may not necessarily be so. Redundent perhaps because food as a word has not been mentioned in this option.
