If possible, try to include everything first n the resume and then look to add something more in the work history form. The adcom will have only your resume with them at the time of your interview so it is imp for the resume to have everything in place in a concise manner.
Try to make it as a one pager as i believe that is the best representation of your skills in a concise manner. Although, i dont seem to remember whether you can make it a two pager, but the one pager has a better effect any day considering that the interviewer doesnt like to glance at the resume more than 3-4 times.
