can anyone explain which is the correct usage "is changed" or "has changed".

IMHO C.

Rather than "is changed" or "has changed" options other than E use "from how it was" unnecessarily.

"has changed little in last century" clearly indicates the action started earlier somtime and continues in present and thus makes "from how it has been/was " redundant
