I just took the GMAT and scored lower than I wanted to (650). I'm signed up to retake the test in 31 days. In the meantime I'm planning on applying to around 25 PhD programs and I'm wondering if I send them my scores now, will I have to pay the $28 per school to send the same schools my updated scores?

I don't want to have to pay twice, but I do want the schools to get my updated scores.

If I don't have to pay twice, then what triggers the GMAC to send my updated scores to the schools that I've already paid the $28 for?

Yes. You will have to pay for each score report.

The scores are not connected which is good on case your next score 550. You would probably not want to share it with all the schools. On the other hand, you do have to pay twice so you may as well wait to order the reports.

Keep in mind that when you send the second score report, your schools will see the first test score. It usually is not a big deal for MBA programs as they will use the score that you prefer to use. Not sure about PhD.
