First she had become ill and then she was discharged.

again do not have OA. but i go with D because she was too ill at the time of discharge.

look for highlighted parts. she was first joined the army not discharged. in A, it is indicated that she was discharged first and then later joined the army.

The year has been specified so their is no ambiguity.
joined the army - No previous action dependency - Past tense

was injured 3 times - No previous action dependency - Past tense

was discharged in 1783 because she had become too ill to serve - Conditional - First action is past perfect (had become ill) and second action (was discharged) in simple past.
