To your points:

"1. how to make it?"

Well it's up to you, but I'd recommend an Excel spreadsheet (easy to save & keep), with columns for date, cause, comments, and rows for question type and any specific item. For instance, my own error log focused on Data Sufficiency and Sentence Correction questions, and so I might have included the area (geometry, alegebra), what threw me (forgot quadratic basics, rushed and missed a step) and ideas to improve (write down parts, remind myself I have time), and so on. You want something simple enough to use, but specific about what seems to be hanging you.

2. when should we start making it?

Start as soon as possible, and try to use it every day. Print out a copy and make notes on it as you think of things. Keeps your mind fresh and helps make the comments useful.

3. how often to update it?

Every time you study. Like any exercise, the more you do it the better you get.

Sorry for the miss devansh_god, I see that bb was able to get you that file.

My dad's still in the hospital, but he's getting back to normal, as in he's complaining about everything - the food, the bed, etc.

Last week I did his taxes for him. If he complains about that, I will know I did them right! :lol:
