Do I need to put a heading on my essay? If so, what information should I list?

Usually the application instructions will specify what headings, if any, are needed. If the instructions don't specify, you're probably ok just putting the essay question at the top of the first page of the essay. Some of the online application systems automatically put identification information at the tops of pages when the applications are printed, so in that case schools may not ask you to put headers.
