(A) overtime that has the potential of causing
- potential of causing is not idiomatic

(B) overtime that has the potential to cause
- suggests a 'special sort' of overtime with the use of 'that' to introduce a modifier (turns it into an essntial modifier). It's now not any overtime, but one that can cause errors

(C) overtime that potentially can cause
- same problem with B

(D) overtime, a practice that has the potential for causing
- 'for cauing' is not idiomatic

(E) overtime, a practice that can, potentially, cause

I'll go with E, though the commas seperating potentially and cause does not seem to be required. Also, the comma between that can and potentially also seem redundant.

- out. seems to indicate overtime has a potential.
(B) overtime that has the potential to cause
- out. seems to indicate overtime has a potential.
(C) overtime that potentially can cause
- out. seems to indicate overtime will cause.
(D) overtime, a practice that has the potential for causing
- out. seems to indicate practice has a potential
(E) overtime, a practice that can, potentially, cause
- right. clearly indicates that working overtime is a practice can cause errors.
