I guess the answer is based on the parallelism between implementing and demanding, thus it should be, "B".

wow it really is a tricky one, spent full 2 mins on finding fault.
eliminating modifiers helped to reduce the clutter.
as...IMF implementing X and demanding Y.

Parallelism works the best here.

For more than five decades, the IMF has been implementing its structural adjustment policies in developing countries as a way toward improved economic outcomes and demanding financial transparency from participating governments.

Hence IMHO (B)

Also,

among
(i) improved economic outcomes, (ii) improving economic outcomes and (iii) economically improved outcomes:

In (i) ' economic outcomes' is the main noun (which are improved) -> correct
(ii) improving is the main noun (or gerund) (economic outcomes is the object) -> incorrect
(iii) 'economically' changes meaning altogether -> incorrect
