Format/ Approach in answering Recommendation Questions

Hi All,

I'm in the process of sending out the links to my recommenders.
I just want to know how they should be doing the recommendation.
I tried to search the web for the details but couldn't get anything

Should they be answering the questions one by one or are they just gonna use these questions as guidelines in doing the letter?
So my question basically is, should the recommendation be in "letter" format or Q&A format?

I'm applying to MIT, UCLA, Insead and Berkeley.

As far as I know, each school will send out a link to the recommender and that recommender will basically go in and answer questions. These are individual questions that recommenders answer and no, it doesn't get put into a 'letter' format (it will remain a Q&A format).
