You are right.

After we had bowed our head, the minister led us in prayer.
It is D.

past perfect tense because we have two actions. bowed is the first action that ended. therefore, had bowed.

past perfect ==> had + past participle.

led is the simple past and is the 2nd past action.

Doesn't that seem redundant though? The word "after" clearly defines when the two past actions took place. That's why I opted for B over D.

I would have picked B but since we already know that that is not the OA ( I wonder why?), my second choice is A.

D can not be the answer as others have clearly mentioned that it is redundant to use 'had' there...
