From your comments it seems like speed, not accuracy, is your problem. You are rushed during the exam and you commit silly mistakes. You say that your fundamentals are fine - I am assuming you deduced that from analyzing the practice tests.
To increase speed, your strategy for Quant and Verbal will be different. For Quant you need to focus on improving speed by using more logical and less algebraic approaches. Dont rush to make equations. Instead, try to use reasoning to solve the questions. It is certainly hard and more so for people trained to assume variables and make equations at the drop of a hat but it is certainly do-able. I myself was a big time algebra person till I met my mentor who taught me holistic approaches. Check out my blog for discussions on logical approaches.

This means start reading extensively - books, magazines, newspaper articles - whatevere you can get your hands on. Read everything with an urgency while making efforts to understand. Reading fast is a skill you can acquire. Also, to keep your accuracy up, use the strategies discussed in our Verbal books on lots of practice questions. Many of these approaches are discussed on our blog too.
