Hello,
i am trying to arrange recommendation letters nowadays. each schhol has downloadable recommendation forms on their web sites. Are these forms necessary to be filled and sent to the school with a recommendation letter? If I can get only the recommendation letter from the recommender and send the letter without recommendation form, do you think it will harm my application?

I don't think it will hurt your application much if you submit a reco letter without the form, but you must make sure that the letter answer all of the questions in the form. Submitting a generic letter (that does not answer the questions) will most certainly hurt your application. This goes back to the point in my other post about making sure that the recommender you choose knows you well (and cares enough to write a complete reco letter!).
