its a hard one... I am leaning towards C since its concise and also "seeks" is correct IMHO - since she is directing "one" of many projects and that one seeks to determine the help that counseling is to the treatment.
D would also be a candidate but it just seems too long and has a "a help" clause that puts me off.

others can you please chime in!?

Its B.
“one of several x “ takes plural verbs
Between A / B.
A : “extent that” is not correct. extent to which.

In an effort improve the quality of patient care, Dr. Lydia Temoscho is directing one of several clinical research projects that seek to determine the extent that psychological counseling helps to supplement the medical treatment of serious disease.

(A) seek to determine the extent that psychological counseling helps to supplement
Incorrect. S-V agreement: one [modifier] seek (plural).
(B) seek to determine how helpful psychological counseling is in supplementing
Incorrect. S-V agreement: one [modifier] seek (plural).
(C) seeks to determine how helpful psychological counseling is to supplement
Correct.
(D) seeks to determine to what extent psychological counseling is a help in supplementing
Incorrect. "is a help" is not as direct as helpful, also "in supplementing" sounds awkward. I am not sure if it is a correct idiom.
(E) seeks to determine the extent that psychological counseling is helpful in supplementing
Incorrect. "in supplementing" sounds awkward. I am not sure if it is a correct idiom.

I'm going with C.

I think it's "A".
C,D,E are off due to the inappropriate number agreement.

The verb "seek" refers to the word "projects" and not to the word "one".

"B" seems not right for me. I have a hard time explaining why is that. Sorry about that.

Anyway, I think that "extent that" is okay and the option "A" is correct.
