I have tried to search a satisfactory answer for this Q. I have not found such an answer so far. However I can eliminate one close contender on some grounds but not for sure.

(A) In + assessing +
Here the sentence opens with a prepositional phrase and uses a participle form assessing as an adjective to qualify a following noun. This calls for a subject of the sentence which is doing the action. You can see that the fragment following the comma does not have a subject.

(B) has subject-verb agreement problem

(C) is grammatically correct and qualifies for an answer
(D) is also grammatically correct and qualifies for an answer

(E) has subject-verb agreement problem

How do we choose between (C) and (D)

I am not very sure. But I know that it is awkward to say
"A question that is irrelevant".
This can be concisely written as
"An irrelevant question is"

(D) has no such problems.

I had a very long discussion with a fellow member on this question. This is what it boiled down to:

Above 2 restrictive clauses could be simplified to a simple adverb clause introduced by the first part of the sentence in D. Read on with adverb clauses here:

Hope I can reason out and/or eliminate the "close but" wrong answers when taking the test on Monday
