In Rubaria, excellent health care is available to virtually the entire population, whereas very few people in Terland receive adequate medical care. Yet, although the death rate for most diseases is higher in Terland than in Rubaria, the percentage of the male population that dies from prostate cancer is significantly higher in Rubaria than in Terland.

Only possible choices are B and C.
B seems more convincing because it says that people in Terland die before they are afflicted by prostate cancer hence have less deaths from prostate cancer.

B. Most men who have prostate cancer are older than the average life expectancy for male inhabitants of Terland.
Can't get prostate cancer if you die because of something else before the age when you can get prostate cancer.
In Rubaria people live longer to the age when you can get prostate cancer, where as in Txxxxx people die earlier than the expected age.

From B we can conclude that old males get prostrate cancer. And not many of them are alive in Terland  And that explains why there is less of this form of cancer among males.
