go w/ D as well.
I think B is incorrect partially b/c it changed the meaning. The damage is serious, but in B, it says the damage is seriously irreversible.

I thought the verb tense was tested here but now I know it was the use of the adjectives that qualify "damage"

serious is modifying damage here....seriously is not the correct form here in the sentence.
For seriously to be correct ,some noun should immediately follow it.
