Plenty of entrepreneurs have succeeded without MBAs. It's far more important to your business's future success that you good be good at designing shoes than that you have a foundation of managment skills. That said, as a secondary priority, acquiring management skills would be a good thing for you to do, and an MBA is probably the most rigorous and recognized way to get those skills. One way to get it without interrupting your career is to pursue a part-time MBA.
As for your arts/graphic design background, that will help you get into B-school, not hold you back, because it will set you apart from the crowd, and B-schools will want you for diversity. You also don't need managing experience per se, though you definitely want to be able to show leadership experience, though it can be from any aspect of your life, including community, extracurricular etc.
