Answer should be 'A'.


Deposit it (Form) with writing sample and 2 references.

Other options look like the form needs to be filled with writing sample and 2 references.

Iam also for A.

But,what about D?

Is there a rule , which asks us to put timestamps at the beginning of the sentence. I was a bit confused about it.

D. By March 1, job applicants must complete both sides of the registration form and deposit it with a writing sample and two references.

here's the explanation:
The original is acceptable but slightly confusing because it separates the adverbial phrase by March 1 too far from the verbs complete and deposit, either of which it could be intended to modify. (C) repeats the original error. (B) and (E) alter the meaning incorrectly by needlessly changing the word order.
