It's a trick!

B has an issue with tense: Not only did it (insert subject) create but also caused.
May work if the order was changed to "It not only created but also caused"

D holds tense and drops the idiom altogether.

I can see why the tense is wrong... made me think. But why is 'but it also' not a part of the idiom anymore?

Cheers!

Just 'but also' is sufficient to state paradox.

'create' and 'caused' is not a parallel structure.

"Not only did SUBJECT create , but it also caused. "

Why would that be wrong ? It is still past tense vs past tense and it correctly uses the idiom . I would say A .

A tricky question to make you fall into trap of not-only but-also idiom usage.

The choice B has correct usage of not only, but also, however, it mistakes by not adding subject to the clause after "but also" making sentence after "but also" orphan

Choice D corrects it by ignoring use of not only.. but also and correctly adding a subject for the end sentence.
