I'm a little confused. Does your recommender have to write a separate recommendation essay in addition to answering the questions? Or is answering the questions the recommendation?

Do people usually waive their right of access? Is there a proper protocol here?

Not sure about your first two questions, but regarding waiving your right to access, the general advice is to do it. If I remember correctly, schools generally have to offer you the right to see your rec letters by law; waiving that right gives adcom the assurance (even if they don't need it) that your recommenders will be absolutely candid, truthful, and not hold back.
