A - ,which refers to settlement - wrong
B - tense ( not sure but doen't sound right )
C- ,which refers to earth -wrong
D- Correct
E- Tense ( was .. is )

Should be E.
In D, ' it ' is can refer to either 'Madagascar' or 'Human settlements'.

You are right.. D is wrong.

I have a question regarding D.
Whether the structure of the sentece is correct if you don't use it ?

Madagascar, one of the last habitable areas of the earth, underwent human settlement, making Madagascar an ideal site for research

Yeah - looks like E.

A B are out due to ambiguous usage of 'which' and 'and that'. C & D change the meaning. Hence out.

(A) 'which' has no clear referent
(B) 'to have undergone' is wrong ==> the last to do something
also 'that' has no clear referent
(C) Madagascar didn't undergo human settlement yet
(D) The same with C
(E) OK

What D means is that undergoing human settlement makes Madagascar an ideal site for research.

D changes the meaning of the original sentence.

According to the actual sentence, Madagascar is one of the last habitable areas that underwent human settlement

But D changes that to - Madagascar was one of the last habitable areas of the earth .
