Thanks for posting your story. I understand your concern. What business schools are looking for, particularly from older candidates, is a record of achievement and accomplishments---someone who is grabbing their vision with both hands. While I understand you now know more about what this vision is and/or should be, you unfortunately have not done much to achieve it yet. Business schools dislike people looking to the MBA as a way to get on track--they prefer to give seats to people who are already on track and have demonstrated they have what it takes to pursue and achieve something impactful. Even though you are already at or past the prime age for b-school, you might be better off spending a couple of years proving you are well positioned to acheive your new vision. I think this would go a long way towards convincing the adcoms you are for real and are not just getting to the end of your rope and now are looking to b-school as a panacea. I think getting on your future track now instead of after b-school will position you far, far better with the adcoms and your story will have real meaning. I also think it's one which will make you stand out---particularly when you are able to tie in all the ways in which your medical training helped your new career move.
Now, on the other hand, if you are committed to going now, you may still be able to make it work, but you'd likely have to settle for a lesser known school. You might also look into part time programs which can be more lenient on and sympathetic towards non-traditional backgrounds.
