Shoud (B) be like
...year and thus costs the nation $50 billion in medical expenses and lost ...
Since the use of "cost" will make "18 million Americans" as the subject, but "costs" will make "Migraine" as the subject. The second one makes more sense since the form of headache costs medical expenses, not the people.
This question is from paper test code 48. How could ETS let out some typo like this?

In your own word, what "it" refer to? There are only two subjects here, either "Migraine" or "18 million Americans".
