pick D because of the word 'costs'...Migraine should be singular (i think!)

'they' in A does not have a clear noun referant. So A is out.

'so as to' is not idiomatic. the idiom is 'so X as to Y' is the idiomatic form. So C is out.

'lost medical expenses' is not wrong so D and E are out.

Where is this from? It doesn't seem like a good gmat question, too many things to consider:
a) ackward
b) do americans cost the nation 50 billion ? that doesn't seem right, we are talking about sickness which is singular and which "costs"; the best usage would be "and thus costing the nation ..."
c) wrong idiom
d) lost medical expenses is not right
e) the same

what's the source?

Me too for B.

A is out because They pronoun is incorrect reference for migraine.
C is out because- SO as to cost nations---sounds vey awkward.
D is out - Lost medical expenses !!!!!!
E is out - WHich thus !!!! doesn't sound sweet together to me also lost medical expenses....Lost is for work time and not for expenses...

However, I have a question, should not "thus" in B be follweed by costing instead of cost ? Any thought on it.
