so as to is incorret in C....

"lost ...expenses" is redundant! ...."expense" itself means sth lost ...we don't need to add "lost " here --> D and E out.
"who" suffices to be the common subject of "lose" and "cost" --> we don't need "they" --> A out.
"so as to" expresses a purpose ---> it distorts the original meaning ---> C out.
B stands.

I agree with the OA, but shouldn't the beginning of the sentence read:
Migraines, the most debilitating common form of headaches, afflict... and thus cost...
Either I am way out in left field or this sentence is dreadfully composed

I too chose B
In D "that" refers to the closest noun ie., the "year" right? or am i missing something?

The costing part refers to the work days lost by the Americans afflicted with migraine and not to the migraine itself. The original sentence defines this point, I guess - "they cost "

Good point, but I still think the sentence is awkwardly constructed using the singular "migrane." This one is debatable...but who cares right? It's not our job to revise the non-underlined portion of the sentence.
