Can you ask your supervisor to refer to this aspect of your review in his letter of rec and exlain the scale?
Unless you are specifically asked a question about integrity, I am leery of submitting an essay saying "Look how honest I am!" I agree with the friend of ours who once told his son, "If anyone ever insists he is honest or you can trust him, hold onto your wallet -- tight." But, if a recommender comments positively on your integrity and trustworthiness in a rec, that's something else entirely and very positive. And clearly if you are responding to a question, referring to this aspect of your company's review process could be relevant.
