If I'd like to include somebody's name in my essay (AdCom member I spoke to and want to refer to this discussion in the essay), should I use Mr Adam Smith or just Adam Smith is fine? If with Mr then how would I know for women whether the lady was Ms or Mrs?

You should be fine with just their full name with no prefix. I would only use a prefix if you refer to them by their last name, Mr. Smith instead of John Smith. However, if you want to add a prefix for a woman and do not know if she is married, always go with "Ms.".
