As far as I remember, there are 2 types of questions: the user generated and the level quizzes. The user generated questions are on the top of your screen, right near "Dashboard" and "Concept Map", you just need to click on "UGE". For the level quizzes I think they become available as soon as you clear some lessons on that level.

Yes thats correct, at the end of every level you have to take a test,once you qualify, the next level gets UNLOCKED.
