I would follow the standard American format:
(a) one page -- remember that people do not have time to read endless pages of your background. We use the two minute rule -- which means this is how much time they spend reviewing your resume.
(b) start with experience
(c) then education
I would not include GMAT, TOEFL and other testing information on your resume as this will be included elsewhere in your application.
This is graduate school -- they do not care about your work in high school -- so do not include information about 10th, 11th, etc.
One page should be the limit -- brevity is appreciated and do not assume they will read past the first page -- no matter how interesting your background is.
