I think 4 . had + past participle for past perfect

Yes. 4.

You are trying to tell the time frame of 2 things that happened in the past:
- Going home
- the remembrance of giving call to Susan

Now to tell the one which happened earlier, you use had.
Since, you reached home first and then you remembered..you are going to say:
'Only after I HAD gone home did I remember I had plans to meet up with Susan for dinner.
