I would go with C on this one...

A - "for monitoring" is incorrect usage
B - generally awkward and meaning is unclear
D - indicates that the route monitors population changes
E - same error as D, with an agreement error as well

C. less wordy. uses "to monitor" and "changes in population" correctly
